Every year, 3,000,000 to 4,000,000 deaf children are born in the world, and only 23% of these children can benefit using cochlear implant systems. Reports from the World Health Organization show that more than 5% of the world’s population in 2020 suffered from hearing loss and require hearing services. Hearing loss is the fourth leading cause of disability in the world.  If these systems are not provided, these children may lose the golden age for surgery due to the high currency cost.

This statistic, in addition to patients in the queue and people who have already been hearing and have lost their hearing due to illness, has created a large market for the needy. Cochlear implantation is a small electronic device that can restore part of hearing to a deaf person by using electrical stimulation of the auditory nerve. This device does not restore normal hearing, but gives the deaf person a new and more efficient form of ambient sounds, enabling them to understand speech properly. The implant includes an external part that is placed behind the ear and also has another part that is placed under the skin during a surgery. A cochlear implant includes the following sections:

  • A microphone, which collects sounds from the surroundings.
  • An Audio/sound processor, behind the ear that receives, selects and organizes the audio/sounds collected by the microphone and converts it to digital code. The audio processor runs on a battery.
  • The sound processor transmits digitally encrypted sound to the implant
  • A transmitter and receiver (simulator) that receives signals from a sound processor and converts and transmits digitally encrypted sound into electrical pulses via an out-of-head coil.
  • An electrode row, stimulate the cochlea and are consists of a group of electrodes that collect pulses from a simulator and send them to different parts of the auditory nerve. The auditory nerve sends impulses to the brain that are perceived as sound in the brain.

 

Why Cochlear Implant Surgery

Cochlear implants improve communication and quality of life for people with severe to profound hearing loss, in which, hearing aids help a little. Cochlear implants in both ears are a standard treatment for severe to profound hearing loss, especially in infants and children who are in the process of learning speech and language. The US Food and Drug Administration (FDA) first approved cochlear implants for the treatment of hearing loss in adults in the mid-1980s. Also, since 2000, cochlear implants have received FDA approval for use in eligible children as young as 12 months. Studies have also shown that eligible children who receive cochlear implants before the age of 12 months have a faster rate of development of language skills similar to children with normal hearing, and in many cases can attend classes successfully.

Many adults who have lost all or part of their hearing during their lifetime can also benefit from cochlear implants. They learn to relate the signals from the implant to the sounds they remember. These sounds include speech without the need for any visual cues such as lip-reading or sign language.

Necessary Pre- examinations for Cochlear Implant Surgery

Procedure of Cochlear Implant Surgery

Cochlear implant surgery is a procedure performed under general anesthesia and takes about 2 to 4 hours. Finally, the incisions are sutured and closed. After cochlear surgery, ‌‌ the patient usually stays in the hospital for one day and returns to normal activities after 3 to 5 days. Mild effects such as mild pressure or discomfort in the corner, vertigo, or nausea from general anesthesia may be experienced after surgery. At the end of the week, a doctor’s examination is required to remove the stitches. To speed recovery, cochlear implants are not activated immediately after surgery, but can be activated 2 to 6 weeks later.

Complications of Cochlear Implantation

Cochlear implantation is usually a low-risk surgery. However, like any other surgery, cochlear implantation may be associated with risks and complications in a low percentage of cases, the most important of which are:

  • Meningitis in children, which is usually done before vaccination to prevent it;
  • Loss of residual normal sense of hearing due to prosthesis placement;
  • Inefficiency of the prosthesis and the need for re-surgery to replace or repair it;
  • In rare cases, complications such as bleeding, infection, imbalance, tinnitus, facial nerve palsy, changes in the sense of taste, and cerebrospinal fluid leakage.

In recent years, one of the concerns for cochlear implant patients (especially those whose prosthesis used a retainer and therefore needed to create a larger valve in the cochlea) was the possibility of developing bacterial meningitis. Today, however, preservatives are no longer used in implant prostheses, and preoperative vaccination and postoperative monitoring have reduced the patient’s risk of any bacterial infection.

Rehabilitation involves training the brain to understand the sounds heard from the cochlear implant. Everyday speech and ambient sounds are heard differently from what one remembers. The brain needs time to recognize the meaning of sounds. This process is improving and is constantly improving with the continuous use of the speech processor during waking hours.

Results after Cochlear Implant

The outcome of cochlear implants varies from person to person.
