What do the different levels of surgeons mean?

Treatment packages are mainly leveled based on the ranking of surgeons (according to the number of annual surgeries, years of experience, educational and experimental records, the rate of successful surgeries and the percentage of patients’ satisfaction) and the hospital/clinic where the surgery is performed.

As an example, level 1 surgeons are Iranian brand doctors who are famous inside and outside the country and are mainly observed by international patients on social networks. It should be noted that all the doctors and surgeons that cooperate with Nora Health Tour are board certified and work in prominent hospitals that are licensed to accept international patients and comply with the standards of Nora Health Tour medical team.

One of the surgeries used to treat retinal and vitreous disorders is called Vitrectomy. To see, light must pass through the eye and reach the retina, which is a bunch of light sensitive tissues behind the eye In fact, the retina sends images, objects, and the environment to the brain in the form of a neural message and sends information to the brain. The retina is located at the back of the eyeball and fills the vitreous in the middle of the eye. Various eye diseases can cause vitreous fluid to become cloudy, fill with blood or debris, harden, or cause scarring. This can block light from reaching the retina and impair vision.

Fluid removal and replacement may solve or improve this problem. The whole or part of the vitreous is drained to remove the opacity of the vitreous and replaced with BSS solution. Sometimes the retina moves away from the surrounding tissue.  In that case; ophthalmologist may perform Vitrectomy to facilitate and repair the retina. Vitrectomy surgery removes blood and debris from an infection that may cause nerve messages to not focus on the retina, resulting in a lack of image. In most cases, vitrectomy surgery strengthens and improves vision or stabilizes and maintains the patient’s existing vision.

Some other problems that Vitrectomy allows to treat are:

  • Damage to blood vessels located in the retina
  • Macular perforation in the retina
  • Intraocular infections
  • Severe eye injuries and surgeries
  • Retinal Wrinkles (Macular Poker)
  • In some types of retinal detachment
  • Treat specific problems, complications, and disorders after cataract surgery.
  • Diabetic retinopathy that results in bleeding and stretching of the retina.

Before Vitrectomy Surgery

Before surgery, ophthalmologist examines patient’s eyes using special tools and light. Doctor might want to do an ultrasound on patient’s eye to look more closely at retina.
The patient’s general health such as heartbeat and blood tests will be evaluated and then she/he will be advised on what pre-operative preparations are needed to make, such as altering medications.

Vitrectomy Surgery Process

Depending on the patient for whom the surgery is performed, Vitrectomy may take from one to several hours. Patient has the choice to stay awake and benefit from anesthetic drops or injections into eyes. Also patient may be under general anesthesia – a medicine that helps and fall asleep during surgery.

Depends on the severity of the complication, all or part of the vitreous is removed from the patient’s eye to reduce the pressure and tension on the retina. If there is blood, the surgeon washes them and replaces the removed vitreous with sterile salt or, if necessary, fluids such as silicone oil and expandable gas bubbles. The purpose of doing this is to keep the retina in its natural position. Also, if the patient also has retinal perforation, laser treatment is performed simultaneously to treat the perforation or to remove the lens opacity in case of cataract.

During the operation due to local anesthesia or general anesthesia, the patient does not feel pain, but it is obvious that after this difficult and complex surgery, the patient needs to use pain killer, which should be prescribed by an ophthalmologist.

After the surgery, the amount of postoperative vision depends on a number of factors, including the severity of the damage to the patient’s retina before surgery. It is very important that the patient understands what harms and dangers to his vision can lead to not performing this surgery on time. Stabilizing the patient’s vision, or in severe cases, having minimal vision is better than blindness.

Vitrectomy is often an irreversible procedure; but the age of the person being treated will have a huge impact on the reversibility of this cataract and the elderly will be at risk.

Postoperative care after vitrectomy surgery

There is no need to stay at the hospital after vitrectomy surgery and patients are discharged from hospitals usually 1 hour after it. After surgery, some activities like swimming have a flight, and driving is forbidden for about 12 weeks. Patient will need 2 to 4 weeks to recover before being able to do normal activities again. It may take longer for patient’s vision to get back to normal.

  • The eyes are painful and sensitive for several days. It is usually necessary to use antibiotic drops and anti-inflammatory drops after surgery. Sometimes other drops, such as antihypertensive drugs or pupil dilators, are also used.
  • After vitrectomy surgery, you should use a shield for 2-3 weeks to protect your eyes from possible blows.
  • Paying attention to the position of the head in a specific position and based on the type of operation (not paying attention to this issue will cause treatment failure.)
  • If a gas bubble is injected into the eye, the person should keep their head down for a week or two and even be in a semi-sitting position to sleep, and should not sleep on their backs in any way. So pay attention to the ophthalmologist’s advice on how to position the head in a certain position, because not paying attention to this issue can lead to treatment failure.
  • After vitrectomy surgery and injectable gas injection, travel by flight is prohibited due to increased eye pressure at altitude, and you must use ground transportation until the gas bubble leaves the eye.
  • In cases where silicone oil is used, it is necessary to monitor, because silicone oil may cause problems over time and there is a need to remove the oil as soon as possible.
  • In general, after retinal surgery, it is necessary to see an ophthalmologist regularly to control the condition of the retina and prevent possible complications.
  • Avoid hitting, rubbing and squeezing the eyes after vitrectomy to speed up the healing process.
  • Use the medication prescribed by your ophthalmologist.
