What do the different levels of surgeons mean?

Treatment packages are mainly leveled based on the ranking of surgeons (according to the number of annual surgeries, years of experience, educational and experimental records, the rate of successful surgeries and the percentage of patients’ satisfaction) and the hospital/clinic where the surgery is performed.

As an example, level 1 surgeons are Iranian brand doctors who are famous inside and outside the country and are mainly observed by international patients on social networks. It should be noted that all the doctors and surgeons that cooperate with Nora Health Tour are board certified and work in prominent hospitals that are licensed to accept international patients and comply with the standards of Nora Health Tour medical team.

Intrauterine Insemination (IUI) is a method of assisted reproduction that may be used alone or with drug stimulation of the ovaries to treat infertility and involves placing sperm inside a woman’s uterus to facilitate fertilization. The goal of the IUI is to increase the number of sperm that reach the fallopian tubes, which in turn increases the chance of fertilization. This option is less invasive and less expensive than assisted reproductive techniques and is the first line of infertility treatment before IVF.

IUI is mostly used in cases of low sperm count or decreased sperm motility. In this case, IUI is an ideal option because placing sperm directly in the uterus increases the chances of fertility. IUI is also used as a fertility treatment for other diseases such as: cervical mucosal problems, IUI puts sperm from the cervix where they can move freely. Also, IUI is mostly used in cases of low sperm count or decreased sperm motility. In this case, IUI is an ideal option because placing sperm directly in the uterus increases the chances of fertility. Beside, IUI is used as a fertility treatment for other diseases such as cervical mucosal problems, where IUI places sperm from the cervix where they can move freely.

After consulting a doctor, a volunteer can determine if they are eligible for treatment. This may include a hysterosalpingogram (HSG) test, which shows that there is an obstruction in the fallopian tube that may prevent the eggs from moving to the uterus during the process. Fertilization Any couple who has passed the HSG test and has a sufficient sperm sample must be eligible for IUI. Totally, IUI is recommended for the patient in case of:

  • Low sperm count or sperm motility.
  • High number of abnormal sperm.
  • Low semen volume.
  • Presence of Anti sperm Antibodies in semen.
  • Fatal sperm secretion in the cervix.
  • Low cervical mucus.
  • In cases where the cervical mucus is very thick and sperm are not allowed to pass. Problems with cervical mucus can be detected during the Post Coital Test (PCT).
  • Mild or moderate endometriosis.
  • Impossibility of ejaculation by a man due to impotence (inability to create and maintain an erection) or structural disorders of the penis, paralysis of the lower extremities or the presence of regressive ejaculation.
  • Existence of ovulation problems in women.
  • Vaginismus (involuntary contraction of the vaginal muscles in a way that makes sexual intercourse impossible).
  • Infertility with unknown causes.

Pre-Operative Care of Intrauterine Insemination (IUI) Surgery

Couples should not have intercourse 72 hours before the IUI, but if low sperm count is the cause of IUI, there should be a 48-hour interval between the last ejaculation and sperm collection, but if there is no problem with the sperm, it should be about 24 hours before There will be no problem with IUI either. After IUI, you can have intimacy at any time, and in fact many doctors recommend intimacy after IUI. Also:

  • Use medication as prescribed by doctor on a regular basis.
  • During medication, 2 to 4 ultrasounds are performed based on the response of the ovaries to determine the appropriate time for ovulation.
  • Take a bath the night before the egg is removed.
  • Avoid face makeup, especially nails.
  • It is better to eat soup the night before the operation and not eat anything after 12 o’clock at night. (Even water)

Also, there are necessary cares for the patient’s husband in giving a sperm sample

  • Do not have intercourse two to seven days before giving the sample.
  • Take a shower the night before giving the sample and wear clean underwear.
  • To give samples; do not use soap, gel, saliva or soft materials.
  • Give the sample in full in the container that clinic has given..
  • If the patient can not give the sample, he will be given a container or device to pour the sample into the container at home and wrap the container in aluminum foil and put it in the pocket of the clothes so that the heat does not decrease and within a maximum of one hour bring to the hospital.

Procedure of Intrauterine Insemination (IUI)

This procedure is relatively simple and painless and is performed without anesthesia. In this procedure, semen is usually washed in a special way and then the washed sperms are inserted into the uterus through a vagina using a narrow tube (catheter).

In this method, in order to increase the chances of pregnancy, by using ovarian stimulant drugs, the number of eggs released in the relevant cycle is increased to two or four eggs. For this purpose, with the order of the Infertility specialist, ovarian stimulant drugs should be taken in exactly the right dose and at the specified time. After examining the condition of the ovaries with the help of ultrasound and observing the follicles and making sure that the condition is normal, the injection of fertility drug (HCG) is performed to stimulate ovulation.

IUI is usually performed about 36 to 48 hours after HCG injection. But sometimes, depending on the doctor and the circumstances, to increase the success rate, IUI is performed twice, at one-day intervals.

After determining the exact time of the IUI operation and announcing it to the couple, the man must deliver his sperm sample to the laboratory in the morning of the scheduled appointment for the operation, after two to five days of abstinence and ejaculation. Sperm samples should be collected by masturbation or using special condoms without spermicides in a sterile container provided by a laboratory. The sample should be fresh and prepared at the laboratory if possible. In the laboratory, the sperm sample is separated from the semen by washing with special materials and ready for injection.

The sperm are then injected into the uterus using a thin plastic tube called a catheter. There is no need to hospitalize the patient after the IUI, but most doctors put the patient to bed for 30 to 60 minutes. The patient does not need absolute rest and can continue his normal daily activities. Two weeks after the IUI operation, a pregnancy test is performed and the result of the operation is determined.

Infertility treatment with IUI is prohibited in if:

  • Women with severe fallopian tube obstruction.
  • Women with a history of pelvic infections.
  • Women with moderate to severe endometriosis

Success Rate of Intrauterine Insemination (IUI) Surgery

The success rate in IUI, depending on the factors mentioned, is between 6% and 24% per treatment cycle. The success of this operation depends on several factors. In cases where the number and quality of male sperm are appropriate, the success rate after using this method is much higher than people who are forced to do so due to low sperm count. On the other hand, gynecological diseases also play an important role in the success rate. The woman’s age and physiological condition play an important role in this regard. For example, if a woman is over 35 years old, the chances of getting pregnant with this method are significantly reduced.

If treatment fails after performing an IUI cycle three to six times, other infertility treatment methods such as in vitro fertilization (IVF) or sperm injection into the cytoplasm of the egg in vitro (ICSI) are used.

Post-Operative Care of Intrauterine Insemination (IUI) Surgery

  1. Normally, after this treatment, there will be no need to be hospitalized and the person can resume her daily activities without any interruption and no absolute rest is required.
  2. Usually, 6 to 12 days after ovulation, embryo implantation can occur in any part of the uterus, and a pregnancy test (βHCG) should be performed through a blood test two weeks after IUI.
  3. If the test result is positive, it is necessary to take the pregnancy test again after 48 hours. This test is done to check the process of increasing pregnancy hormone (βHCG).

General Recommendations to Increase the Success of IUI

  1. Having sex after IUI increases your chances of having children; Provided that the patient does not have spotting or bleeding during IUI, in which case sexual intercourse takes place 48 hours after IUI.
  2. If you use suppositories such as those commonly prescribed after IUI, you should avoid swimming after IUI, otherwise there are no restrictions on swimming.
  3. It is best to start taking pregnancy supplements, such as vitamins, the day after IUI.
  4. Having a good diet, exercise, non-smoking and alcohol and reducing caffeine consumption in women increase fertility success.
  5. In men, having a healthy diet rich in antioxidants (such as vegetables and fruits), taking multivitamins, keeping weight in the proper range, exercising regularly, avoiding stress (which interferes with essential sperm-producing hormones), and ensuring Non-toxicity to heavy metals, insecticides and chemicals in solvents, which can affect the quality and quantity of sperm, will affect the success rate of IUI.
  6. No smoking in men; Smoking affects sperm by the following two mechanisms:
  • It leads to the deformation of sperm and as a result, disorders related to the penetration of sperm into the egg occur.
  • It destroys the DNA of sperm. DNA damage leads to serious problems (such as developmental-mental retardation) in the fetus. Even chewing tobacco has been shown to reduce both sperm count and erectile dysfunction in the penis.
  1. Men refrain from bathing in hot water or hot baths or even wearing tight clothing that restricts blood flow to the testicles and thus affects the health of sperm.
  2. Taking certain antibiotics can affect sperm quality, so you should consult your infertility doctor about not taking these antibiotics for a few weeks before your IUI.

Postoperative Complications of IUI

Vaginal discharge after IUI is very common and quite normal and has nothing to worry about. This discharge is not an IUD leak, but cervical mucus that separates during the operation and now comes out of the vagina. Many women who have had an IUI have had pain similar to a Pap smear. However, passing a catheter through the cervix can cause spotting, bleeding, and abdominal pain due to the possibility of minor injuries. Partial spotting after IUI surgery is also normal. Heartburn after IUI surgery can be relieved by taking over-the-counter painkillers. A handful of patients experience severe pain, bloating, or bleeding, in which case a physician should be contacted immediately, as these are signs of infection or other rare side effects.

Frequently asked questions

FAQ

WHO ARE GOOD CANDIDATES FOR IUI?

IUI is used when the patient has a small number of sperm, as well as semen volume, antisperm, antibody in semen or even a large number of sperm. Women with conditions such as vaginismus, endometriosis, cervical discharge, or even ovulation problems are some of the things that women need to have an IUI to treat infertility.

AT WHAT AGE WILL IUI BE SUCCESSFUL?

Between the ages of 20 and 25, IUI results will be more successful than those of older people, thus reducing the chances of having children as you get older.

WHAT IS THE SUCCESS RATE OF INFERTILITY TREATMENT WITH IUI?

The success of IUI depends on several factors. Depending on variables such as women’s age, the cause of infertility and the use of fertility drugs and other variables reach 20% per treatment cycle. Because this is a less invasive and cheaper option, the pregnancy rate is lower than IVF.

WHAT ARE THE SIDE EFFECTS OF INFERTILITY TREATMENT WITH IUI?

If you take fertility drugs while taking IUI, you are many times more likely to get pregnant, and you are less likely to get an infection after treatment.

DOES HAVING AN IUI HURT?

Most women who have an IUD take a color photo of the uterus beforehand. IUI pain is by no means the same as a color photo. It can be compared to a Pap smear! Of course, before and after the IUD, mild abdominal pain is normal due to the entry of sperm and possibly medication.

HOW MANY TIMES CAN AN IUI BE PERFORMED?

Usually 3 courses of IUI surgery are prescribed, which in certain circumstances, the number of these courses may increase. But at least three periods are required to get the answer. However, many patients get results the very first time and become pregnant.

WHEN IS THE BEST TIME TO DO IUI?

It is best to do an IUI about 6-12 hours before ovulation. Approximately 36 to 48 hours after Hcg injection. However, in cases of infertility due to male problems, some doctors believe that the best time to perform an IUI is after ovulation.

DO I HAVE TO REST COMPLETELY AFTER HAVING AN IUI?

Not at all. Most doctors, however, prefer to stay in a supine position for half an hour after an IUI. But there is no need for absolute rest at home. It is just better not to do heavy activities.

DO SPERM COME OUT AFTER IUI?

No, if you feel dampness and even discharge after the IUI, it is often due to the hormonal status of your body and the arrival of a catheter, etc.

IS BLEEDING AND SPOTTING NORMAL AFTER AN IUI?

Spotting is common after an IUI. But if the amount of bleeding is more than a simple bleeding, be sure to tell your doctor.
