What do the different levels of surgeons mean?

Treatment packages are mainly leveled based on the ranking of surgeons (according to the number of annual surgeries, years of experience, educational and experimental records, the rate of successful surgeries and the percentage of patients’ satisfaction) and the hospital/clinic where the surgery is performed.

 As an example, level 1 surgeons are Iranian brand doctors who are famous inside and outside the country and are mainly observed by international patients on social networks. It should be noted that all the doctors and surgeons that cooperate with Nora Health Tour are board certified and work in prominent hospitals that are licensed to accept international patients and comply with the standards of Nora Health Tour medical team.

When couples encounter the phenomenon of infertility, their first action depends on various infertility treatments. Some couples may mistakenly think that they are infertile due to lack of awareness of this phenomenon and seek different treatments in line with this belief, so first you need to know what infertility is basically and with what symptoms. The next step, after diagnosing infertility, is to treat infertility with sufficient awareness. Infertility actually means having sex with the body without any care for a year and infertility. Therefore, couples who refer to a gynecologist after having a relationship with infertility for 2 to 3 times; They should be aware that this is not enough time for diagnosis and that they may experience fertility if they do not have a specific problem with medication. As you know, infertility is not limited to women. In fact, it may sometimes be due to female infertility, lack of quality and lack of sufficient sperm in men.

One of the most common questions that couples ask a gynecologist in infertility treatment is that if it is the best treatment. We must say that the answer to this question depends on the cause of infertility in them. Perhaps the first word they hear in these centers is IVF (In Vitro Fertilization). IVF is usually used as the first line of treatment and the best treatment for bilateral obstruction of the ovarian tubes. IVF (In Vitro Fertilization) procedure was first performed in 1978. Simple fertilization of a female egg with male sperm in the laboratory is IVF. After fertilization, the embryo is cultured in a laboratory for a certain stage and then the embryo is inserted into the uterus through the cervix. One of the best ways to treat bilateral obstruction of the ovarian tubes is to have an IVF procedure. IVF treatment is usually needed when a person with problems such as ovulation disorders, unknown causes of infertility, fallopian tubes and low sperm count in men can use this method. This treatment, which lasts approximately 4 to 6 weeks, consists of 7 steps:

  • Stimulation of ovulation: This stage is similar to the ovulation stage in IUI, in which different methods are used to stimulate ovulation.
  • Follicle growth: The specialist will use an ultrasound or blood test to check the growth of the follicles after ovulation. If it grows properly, the next step will be done.
  • Egg maturation: At this stage, the doctor injects the eggs to their final maturity so that they can be fertilized with sperm. This will take about 36 hours.
  • Ovulation or puncture: By examining the eggs by ultrasound using a special needle, the eggs will be separated from the follicles. At this stage, about 8 to 15 eggs will be released.
  • Fertilization: After the egg is removed, the sperm received from the man in the laboratory will be placed next to each other with the egg. Sometimes microinjection is used to increase the chance of fertilization, in which sperm is injected directly into the egg.
  • Embryo transfer: After 3 days, the fertilized eggs will have about 6 to 10 cells and after the fifth day, they will be ready to be transferred to the uterus, which will be transferred into the uterus through a narrow tube.

IVF is used for infertility for the following reasons:

  1. Severe tubular disorders such as hydrosalpinx, fallopian tube obstruction, or pelvic adhesions;
  2. Severe endometriosis;
  3. Failure in two to four cycles of ovarian stimulation with IUI;
  4. Causes of infertility in men (decreased sperm count or motility);
  5. Ovulation disorders, decreased ovarian reserve (reduced number of eggs) ;
  6. Age-related infertility (over 38 years) ;
  7. Infertility with unknown causes.

Pre-Operative Care of IVF

Prior to IVF, patients will have a consultation with their doctor. Both couple’s tests need to be done so that the doctor can be sure that there is no infection in either of their reproductive systems and both are ready to do so. At this appointment, the doctor will ask both of them if they have tried IVF before, and then explain to them in detail about IVF and the risks involved. And will inform them of the success rate. Other tips that should be followed before IVF surgery:

  • Tell your doctor if you have previously taken medication for infertility.
  • Tell your doctor if you have had any surgery before.
  • A person’s BMI is effective in this operation, so the patient should lead an active life and have a proper diet before IVF.
  • Caffeine intake should be controlled as it can affect the success rate of the operation.
  • If you decide to treat infertility through IVF, you should both quit smoking and drinking alcohol.

IVF involves several complex and difficult steps, so couples who decide to do this should be mentally and emotionally fully prepared for such a process. You will fill out the consent form before starting treatment. You and your partner also have blood tests for HIV, hepatitis B, hepatitis C and lymphotropic T cells. Before IVF, women have their first ovarian test. This test involves taking a blood sample and testing it for follicle-stimulating hormone levels. The results of this test give your doctor information about the size and quality of your egg. The doctor will also examine the uterus. The test involves an ultrasound that uses high-frequency sound waves to create an image of the uterus. It also measures your blood flow to the ovaries and determines your fertility status. The doctor can also insert a marker through the vagina and uterus. These tests can show the health of the uterus, check for implantation of the fetus, and help the doctor choose the best way to fertilize.

 Men need sperm testing. This test involves using a semen sample that the lab analyzes for sperm count, size and shape. If the sperm is weak or damaged, surgery called an intracytoplasmic sperm injection (ICSI) may be necessary. In ICSI, the technician injects sperm directly into the egg. ICSI (Microinjection Pregnancy (ICSI)) can be part of the IVF process.

Duration of IVF Treatment

Regarding the duration of IVF treatment, it can be said that it takes about 3 weeks from the start of prescribing and receiving drugs to the transfer of the fetus into the uterus. But before the start of IVF treatment, from the first visit and consultation, review of medical records and records to ultrasound examinations and tests necessary to start the IVF process, it will take about 4 weeks.

After these steps, IVF treatment is started and fertility drugs are prescribed for about two weeks. In fact, this period (one month) is influenced by sex hormones in a woman’s body in a small bag the size of a grape seed that contains an egg is growing. During this period, the necessary tests and ultrasound are performed to check the condition of the follicles and the uterus, and after these two weeks, an ovulation or puncture operation is performed, which lasts about half a day. After that, the egg and sperm are fertilized in the laboratory and about 3 to 5 days after fertilization, the embryo is examined for transfer and then the embryo is transferred. About two weeks after the end of IVF treatment, a beta test, or pregnancy blood test, is performed in two stages to confirm pregnancy.

IVF procedure

IVF operation involves several stages that make up an IVF cycle. The first step is to stimulate the ovaries. This process takes 8 to 14 days. During this period, your doctor will prescribe drugs that raise the normal levels of the hormones FSH and LH to stimulate the ovaries to produce more eggs. This drug often needs to be injected into the body, which is why this stage is called “IVF injection”. The patient injects this drug into his body according to his physical condition. The injection is given once a day or once or twice during a cycle. The doctor will tell the patient which method is best for him. At this stage, the condition of the ovaries is checked through a blood test or transvaginal ultrasound. The last injection is very important because after that the eggs must be ready for fertilization. The fertility specialist determines the ovulation schedule before the ovulation period.

The second stage of infertility treatment is through artificial insemination and egg retrieval. This is done at a hospital or doctor’s clinic. At this stage, the doctor removes the eggs from each ovary with a scanner controlled by the latest ultrasound technology. This will take at least 30 minutes. After doing this, the patient will rest for 30 minutes and will be ready to walk and return home. If you decide to use fresh sperm, a sperm sample should also be taken on the morning of egg retrieval.

In the third step, the collected sperm and eggs are placed in a container and combined with each other, and a kind of in vitro fertilization takes place. If the sperm can fertilize the egg, it becomes an embryo. The doctor then keeps the fertilized egg for 5 to 6 days and monitors it for growth conditions.

Finally, the fertilized egg is injected into a woman’s uterus. This only takes 5 minutes and does not require any anesthesia. The patient can return home after doing this.

About 12 days or two weeks after the fertilized egg is injected into the uterus, the doctor takes a blood test from the woman to see if she is pregnant. If you are lucky, the fertilized egg implanted in the uterus will grow and form a complete embryo. If more than one fertilization occurs, you are more likely to become pregnant, but you are also more likely to have twins or more. That is why about 20% of babies born through in vitro fertilization or IVF are twins, triplets and even more.

Possible Inconveniences

Abdominal cramps or a feeling of fullness in the bladder may occur after the egg is removed, which is normal. You may also have headaches, chest swelling, cramps, pelvic pain, and mild bloating after the egg is injected or implanted.

Chance of Pregnancy in IVF

The chances of succeeding in IVF will vary depending on the person’s age, fertility problems, previous pregnancy, lifestyle and treatment. The younger a person is, the better their chances of getting pregnant in IVF. People under the age of 35 have a 20 to 30 percent chance of succeeding in IVF, and the rate decreases with age. To increase the chances of pregnancy in IVF, methods such as fetal freezing, endometrial ablation, egg hatching and endometrial PRP can be used according to the doctor.

Complications of IVF

Some complications of IVF include:

  • Excretion of a small amount of clear or blood-stained fluid after the IVF procedure, mild abdominal cramping,
  • Mild bloating, constipation, tenderness (sensitivity) of the breast.

Symptoms that you must inform your doctor if observed:

  • Severe vaginal bleeding, pelvic pain, blood in urine and fever above 38 ° C
  • Some of the side effects of IVF are related to the drugs that are prescribed during the IVF process to treat infertility and include headache, abdominal pain, hot flashes, bloating, and ovarian hyperstimulation syndrome.
  • The most important symptoms of drug-induced ovarian hyperstimulation syndrome are nausea or vomiting, decreased urination, lethargy and fainting, severe abdominal pain and bloating, and weight gain over three to five days.
  • Other complications of IVF include an increase in multiple births due to the transfer of multiple embryos to the uterus during the IVF process, and therefore the risk of preterm delivery, low birth weight, and common complications in multiple pregnancies also increases.

Post-Operative Care of IVF

  1. Do not drive after IVF. It is best to have someone with you to take you home after the egg is harvested and implanted.
  2. After the embryo transfer, rest preferably for 24 hours. Relax completely in bed and just get up for bathing and eating and then start your daily activities.
  3. Avoid strenuous physical activity such as running, aerobics, or sports such as football or tennis.
  4. Do not swim or bathe for 48 hours.
  5. Avoid using tampons and vaginal wash.
  6. Doing heavy work, lifting heavy objects, strenuous physical activity and exercise is not recommended until the pregnancy is stable.
  7. Avoid having sex for a few days before IVF and two weeks after a fertilized egg injection.
  8. Adjust your diet so that you do not get constipated or bloated.
  9. You may notice some discharge or spotting after IVF. Mild swelling and tenderness when touching the breasts as well as mild muscle cramps are normal and need not be a concern. But if you have moderate to severe pain or heavy bleeding, be sure to call your doctor.
  10. Two or three days after the operation, the patient can easily travel by air. However, depending on the patient’s condition, your doctor may recommend that you rest for a few days after the egg is implanted.

Frequently asked questions

FAQ

DOES IVF DAMAGE THE OVARIES?

Normally, IVF, egg removal, and laparoscopy do not damage the ovaries, but there is some evidence that breast biopsies can damage the ovaries.

WHAT IS THE FERTILITY RATE WITH IVF?

Most couples believe that the chance of getting pregnant by IVF is one hundred percent. The chance of fertility in any normal couple without infertility is about 15-20% per cycle. In the best infertility centers in the world and infertility treatment centers in Iran, using assisted reproductive methods such as IVF, this rate in each cycle will be a maximum of 40-35%, which provides a relatively high condition and chance for fertility of couples.

IS IVF PREGNANCY DIFFERENT FROM NORMAL PREGNANCY?

Women who become pregnant with IVF should be more careful than with a normal pregnancy, but this does not mean complete rest and restriction of daily activities unless you have a specific obstetric problem that your doctor recommends absolute rest. It is better to be under the supervision of one of the best gynecologists and infertility doctors, to have more rest and if you are employed, to avoid long working hours or heavy activity. If necessary, your doctor may prescribe anticoagulants, etc., which you should use according to the instructions. Of course, prenatal care will be special for people who are pregnant with EVF.

ARE IVF CHILDREN HEALTHY IN EVERY WAY?

Some couples think that in the IVF method, and especially in the PGD (for sex determination), the fetus should be healthy in all respects. It is true that during embryo transfer, the best and highest quality embryos are selected and transferred to the uterus, but the rate of fetal abnormality, fetal and infant health are structurally and genetically the same as the general population and normal pregnancy. Fetal health tests and screenings will be performed as well as normal pregnancy conditions for the IVF fetus.

IS THE ABORTION RATE HIGHER IN IVF?

When the fetus is implanted in the uterus and pregnancy occurs, the continuation of the pregnancy will be as normal as during pregnancy. That is, the rate of miscarriage, ectopic pregnancy, preterm delivery and all the conditions that may occur in a normal pregnancy and turn the pregnancy into a high-risk pregnancy may also occur in these cases. But do not worry, the rate of miscarriage in IVF is higher It will not be from others.

WHAT IS THE SUCCESSFUL EVF RATE IN THE FIRST CYCLE?

Some couples think that IVF pregnancy is guaranteed and if they do not, they will be disappointed with continuing treatment. Some people think that IVF is not successful the first time and should be done several times. It is natural that the condition of each couple and the quality of the embryos affect the fertility rate, but as mentioned, the success rate of IVF in each cycle in the best conditions is 35-40%, which provides a very good chance in the treatment of infertility in couples. If the pregnancy is not the first time, if there is a frozen fetus for that couple, they can try to transfer the fetus to the uterus again in the next cycles at a lower cost.

IS IVF THE BEST TREATMENT FOR INFERTILITY?

Among assisted reproductive therapies, IVF and microinjection provide the greatest chance of fertility for couples. But this does not mean that this method is suitable for all couples or the pregnancy rate is maximum for all couples. When you go to one of the best infertility doctors for advice and treatment, the doctor, after evaluation and according to the condition of each couple, offers them the best method of infertility treatment. For some couples, for example in cases of severe male infertility, the need For donated eggs, rented uterus, etc., IVF is the first method of treatment, and some are treated with simpler treatments such as ovarian stimulation with drugs or IUDs, and if you are not pregnant, the last line of treatment is IVF is offered to them.
