Facelift Surgery

Rhytidectomy

When ageing process starts, the skin of the face and neck loosens and wrinkles on the cheeks, jawline and around the lips increase. Every year, millions of people use different lift methods to improve the appearance of their skin to create a younger look and eliminate sagging and wrinkles are to lift the skin of the face and neck (platysmaplasty). Facelift and tightening of the facial skin known as rhytidectomy is one of the beauty methods, which stretch the facial skin upwards. During this procedure, fat and dead skin cells are removed and skin cells are stimulated to regenerate. In this case, the sagging of the skin in the cheeks and jaw line is eliminated and it rejuvenates the skin.

Some people have done a face lift along with a neck lift for a better effect and have been satisfied with these changes. So, one of the most effective ways to improve the effects of aging will be neck lift beside facelift. In neck lift, by lifting the skin and removing fine and deep wrinkles, the skin rejuvenates and finds a desirable appearance. The first signs of aging are seen in the neck area. The sagging of the skin in this area, wrinkles and loss of flexibility are somewhat improved by neck and face lifting methods.

A long-term study published in 2014 found that 68.5% of people who underwent a facelift over a 12-year period were completely satisfied with the results. Satisfied or exceeded their expectations. About the same number of people believed that they were 10 years younger. The best candidates for skin lift are people who have realistic expectations of this cosmetic procedure and can fully follow the doctor’s instructions before and after the lift.

Factors affecting the skin

Wrinkles on the neck and face can occur for a variety of reasons.

  • Age increasing
  • Prolonged exposure to sunlight
  • Weight Loss
  • Living and environmental environment
  • Stress
  • Tobacco and alcohol use
  • Inheritance and Genetics
  • Gravity of the earth

Suitable candidate for facelift Surgery

In general, facelift surgery is suitable for people who have a lot of wrinkles and loose skin.

  • People who have deep wrinkles from the side of the nose to the side of the mouth
  • Deep wrinkles on the cheeks and loose skin on the cheeks
  • Loose skin, creases and vertical columns or excess neck fat

Different Techniques for Facelift

Face and neck lifts are performed in different ways. Each of these methods is applicable to specific people. Facial lift or facelift procedures are different depending on the degree of skin imperfections.

  • Non-surgical procedure
  • Surgical procedure

In the past, facelifts were only possible through surgery .Non-surgical rejuvenation treatments may not produce the same results, but they can delay the proper time for a facelift, or complement the surgical results. , using various non-surgical methods, lifting can be done with minimal pain and without recovery  period. Non-surgical rejuvenation treatments may not produce the same results, but they can delay the proper time for a facelift, or complement the surgical results. Of course, non-surgical procedures are temporary and should be renewed from time to time. Non-surgical procedures do not require anesthesia, hospitalization, or deep incisions. These methods generally help the skin to collagen and stimulate skin cells.

Benefits of Non-Surgical Facelift

As you probably know, each method has its advantages and disadvantages. It can never be said that it is a flawless method, but it can be acknowledged that a method can have more advantages than disadvantages. The most important benefits of non-surgical face lift methods are:

  • Possibility of simultaneous neck and face lift
  • Has a short recovery period
  • No pain and bleeding
  • No anesthesia

Variety of Non-Surgical Facelift Procedures

Depending on the skin condition and needs of each person, different methods can be used to increase the beauty and rejuvenation of the face. The different types of non-surgical face lift procedures include the following. The choice of these methods is up to the doctor and will be done after diagnosing the amount of sagging facial skin and the required rejuvenation. Usually, the cosmetic doctor or plastic surgeon uses combined methods according to the client’s age, skin quality, health status and previously used treatment methods.

Hyphutherapy (HIFU)

HIFU is the most advanced technology approved by the FDA and uses ultrasound to target the underlying layers of the skin. The energy of the ultrasonic waves causes the targeted tissue to heat up very quickly. When cells in the target area reach a certain temperature, they experience cell damage. Although this can seem paradoxical, the damage done stimulates the cells and ultimately produces more collagen and rejuvenates and lifts the skin.

This method is more expensive than other non-surgical lift methods. The result of HIFU therapy lasts between a few months to a year. Only one treatment session is required. Its side effects are short-lived and there is a possibility of redness and swelling. In rare cases, bruising and numbness may be felt on the face for several days.

Lifting the skin with the help of HIFU therapy is a safe and effective treatment for people who want to reverse or eliminate the symptoms of aging. This method focuses on rejuvenating the deep structural layers below the skin surface. HIFU therapy can effectively improve the quality and appearance of the skin and thus increase a person’s self-confidence. By reconstructing the underlying layers, long-term effects on a person’s skin are achievable, and compared to routine facelift surgery, this procedure eliminates the need for invasive procedures without the need for long rest periods.

Microneedling

Microneedling (CIT) is a method of removing wrinkles, pain and invasions without bleeding, pain and invasion. That’s why we consider it one of the non-surgical methods to achieve a beautiful face. In fact, microneedling is also known as a collagen induction therapy. This treatment may help people who are looking to reduce acne scars and symptoms such as stretch marks (decreased skin elasticity) in different areas of the face.

In this procedure, skin sterilization and collagen production are performed using sterile microneedles to send energy and heat to the underlying layers of the skin. The main advantage of microneedling is its high effectiveness while low invasion and its negative point is that it does not treat severe sagging skin and is suitable for superficial skin problems. Doing microneedling helps to stimulate collagen production by eliminating complications such as wrinkles, scars (pimples and acne) and injury, stretch marks (skin cracks), superficial burns of the skin of the face and body, darkness and various spots on the skin of the face and body, and etc.

Finally, combining microneedling with other treatments helps patient to get best results. The combination of microneedling method with filler injection is effective in relieving the symptoms of stretch marks on the thighs and abdomen. Many studies have shown the effectiveness of microneedling with PRP (PRP + Microneedling) topically in skin rejuvenation, which has yielded excellent results. Also, one of the main treatments that can be combined with microneedling is mesotherapy, which brings brilliant results.

Microdermabrasion

Microdermabrasion is a method that is used as an uncomplicated and invasive solution to exfoliate the face and remove dead cells. In this method, using a special device and mechanical method, a surface cut is created on the skin, and thus, with the onset of suction, blood flow increases and as a result, the amount of collagen production increases. The result of microdermabrasion is rejuvenation and increased skin freshness.

Fractional RF

Fractional RF, also known as skin tightening with radio waves, is a non-surgical face lift method that helps improve the structural condition of the skin. This treatment uses energy waves to warm the deep tissues of the skin, known as the dermis or dermis. This heat stimulates the tissue to produce more collagen. Radiofrequency therapy has been used since 2001 to combat sagging skin and the symptoms of aging.

Facelift with thread

Facelift with thread is a procedure that is very similar to facial surgery or lifting facial skin. In this process, the skin structure is restored and improved, so you do not need to use a surgical tool or knife to do this. In this method, yarn fibers are used to rejuvenate and pull the skin upwards, these fibers are guided into the skin with very thin needles to keep the tissues and skin high and improve the appearance of the skin. The benefits of a floss face lift include:

  • The effects of treatment do not diminish over time.
  • No extra special care is required.
  • The most important point of this method is its high safety.
  • This method does not require any special preparation.
  • The effects of treatment are immediately visible.
  • There are no injuries or wounds on the skin.
  • This method can be used for people 20 years and older.
  • The strings used are not visible or tactile. Also, these disciplines are very durable and their effects do not disappear.
  • If necessary, it is possible to repeat this operation in the same place or other places on the face and body in the following months and years.

Prior to flossing, patients receive local anesthesia in the area they want to lift (usually in the middle of the face, chin, or neck). The dermatologist inserts a pre-threaded needle into the subcutaneous surface of the facial tissues, meaning that the needle is inserted under a layer of skin that is visible. After loading the yarns in the desired areas, new collagen and fibrous tissue is formed around the yarns, which keeps the texture and tightens the skin. The lifespan of the threads is about 1 year and you can extend the skin upwards by renewing them.

Endolift

Using endolift laser for facial skin lift is a new way to stimulate deep tissues to make new collagen to reduce sagging skin and tighten it. In this procedure, a very thin fiber is inserted into the skin, and then placed between the deep dermis and adipose tissue, and the diode laser is connected to this fiber through another fiber. By moving the fiber in this area, the laser energy the diode is emitted in the infrared spectrum.

Fat and Filler injection

Fat and gel fillers can help increase skin elasticity by increasing the volume of the subcutaneous tissue. Of course, it is better to perform fat injection and gel injection in a way that is simultaneous and in parallel with other methods in order to get a suitable result.Because fat transplantation uses the body’s own fat cells instead of a synthetic gel, these materials are more effective and durable than gels. However, some people prefer gel injections due to the higher cost of fat injections and suction in this method.

The durability of the skin filler depends on the gel brand and body type. But gels usually last between 6 and 12 months. Ask and research the Filler brand when consulting your dermatologist, hair and beauty doctor. Fillers must be FDA approved.

Botax injection

Botox is a substance that is injected intramuscularly. By reducing the contractions of various facial muscles, this substance can help eliminate wrinkles and thus lift the facial skin. Botox blocks the nerve signal to the target muscles. The injected muscle can no longer contract to the muscle through the pulse generated by the nerve, and this causes the wrinkles to relax and not contract and rejuvenate the face. Botulinum toxin is used by paralyzing subcutaneous muscles to smooth out facial wrinkles and make people look younger; Such as frown wrinkles that appear between the eyebrows or on the forehead and wrinkles around the eyes and mouth that appear when laughing. Botox injections are sometimes used to reduce sweating in the palms of the hands or underarms.

PRP

It is a substance that is taken directly from a person’s own blood. PRP injection means extracting blood platelets and using them for rejuvenation and facelift. One of the uses of PRP in rejuvenating and removing wrinkles on the face, laugh line and around the eyes and neck is to deal with the signs of aging naturally. PRP is an effective and safe treatment. Many people nowadays use platelet-rich plasma for facial rejuvenation, as it is a very good treatment without side effects that leads to both long-term and short-term results, as long as the body has growth factors. Continue and use them to create new and healthy tissue.

Carboxytherapy

In carboxy therapy, CO2 gas is released in the middle layers of the skin. In this case, the oxygen supply in that area is reduced. Lack of oxygen stimulates the brain and delivers blood and nutrients to the desired area. This process causes the formation of more collagen and skin rejuvenation. Depending on the skin type, your cosmetologist may continue this treatment for one to several sessions per week for several weeks.

Facelift Surgical Procedure

Ritectomy is derived from the two Greek words “retis” meaning “wrinkle” and “ectome” meaning “surgical incision” and means the removal of facial wrinkles. Facelift is a type of cosmetic surgery by which the fat of the face, cheeks and neck can be shaped or moved and the underlying tissue placed in a new position. Usually the deeper layers of the face and muscles are also lifted. The skin is pulled back on the lifted areas and the excess skin is removed. Facelift incisions are located at the front and back of the ear. Another incision under the chin may be needed to improve neck aging. Sutures or skin adhesives are used to close the incisions. Hanging lips, loose skin on the neck and accumulation of fat under the chin can be corrected with a neck lift. The neck lift usually begins at the front of the ear and wraps around the back of the ear and can continue to the hairline.

Lifting the skin is an outpatient surgery that begins with local anesthesia. The surgeon first injects a sedative and anesthetic into your skin to numb them. Of course, some surgeons use general anesthesia; because the amount of relaxation of facial muscles is more in the state of general anesthesia. Surgery time varies between 2 and 6 hours. It depends on the amount of wrinkles and fat on your skin.

Depending on the skin condition and needs of each person, plastic surgeon chooses among several facelift surgical methods. Depending on the type of skin and the amount of facial wrinkles, lifting can be done on the whole face or part of your face. For example, people who have only wrinkles on the forehead and around the eyes, the operation is performed for them from the hairline to the ears; But if you have wrinkles in other parts of the mouth, such as around the mouth and cheeks, in addition to eye wrinkles, the operation should be performed on the whole face. In the lifting operation, your face will be segmented and incisions will be made on the skin with great care.

Types of face lifts

  • Full face Lift: Complete face lift has comprehensive results. With this operation, the doctor can treat wrinkles under the eyes, sagging around the cheeks, lines around the lips, and excess skin under the chin, double chin, etc. This operation covers a large part of the face. The incisions that are made are longer and larger and are made along the temples and around the ears to the neck. A full face lift can greatly alleviate the signs of aging. This practice is most popular among people between the ages of 50 and 60.
  • Lift the middle part of the face: Using this method, the surgeon can pull and Lift the tissues under the eyes and around the cheeks. To do this, the plastic surgeon makes incisions similar to full face lift incisions, except that the incisions are deeper. With this technique, the surgeon can Lift the muscles that remain intact in full Lift. Since this operation cannot eliminate all the signs and symptoms of aging, it is suitable for people who are in their 30s and 40s and just want to look younger.
  • Mini face lift: The incisions made in this operation are smaller and have a shorter recovery period. A mini-face lift, just like a mid-face lift, treats smaller areas such as the jaw, lower cheeks and neck. In this procedure, the surgeon makes incisions along the hairline and behind the corners, lifting the muscles to remove unwanted fat and excess skin. Many surgeons perform mini-facelifts using the endoscopic technique. This procedure, like the mid-face lift, is common among younger people who have few signs of aging on their face.
  • Lift the lower part of the face: As the name implies, this action targets the lower third of the face. This is a great way to treat chin, wrinkles between the nose and lips, lift drooping corners of the lips and improve jaw lines. The procedure for lifting the lower part of the face is similar to other types of face lifts. In this operation, the surgeon makes incisions along the hairline and around the corners. This cosmetic surgery is suitable for all ages and people who want to remove the signs of aging from their face and have a younger face can enjoy the benefits of this operation.

Different techniques Facelift Surgery

Endoscopic Facelift Surgery

If the person is elderly and needs to lift all parts of the face, a full lift is performed. But for relatively young people with fewer wrinkles, the plastic surgeon recommends a mini face lift. In this method, the incisions are small and can lift the middle part of the face (for example, the cheeks) and the upper part of the face (ie the forehead) with a small cut, but is not able to lift the lower part of the face. The whole face cannot be lifted with this method.

Open Facelift Surgery

Surgery time varies between 2 and 6 hours. It depends on the amount of wrinkles and fat on your skin. After the incision is made, dead skin cells and wrinkles are removed along with the fat, which eliminates the wrinkles on your face. After the surgery, the cosmetologist closes the incisions in your skin with small stitches. These stitches are very small and after complete healing, no trace of them will remain on your face.

Pre- examinations necessary for Facelift Surgery

Plastic surgeon will take photos of patient face from different angles. He/She will also examine bone structure, shape of face, fat distribution and quality of skin to determine best options for facelift surgery. Plastic surgeon will ask questions about patient’s regular medications, history of smoking, drug or alcohol use, and his/her expectations for the outcomes of a facelift. Also, preoperative tests include blood test is necessary.

Pre-Operative Care of Facelift Surgery

  • Follow medication directions and take all your medical records to the clinic
  • Wash your face and hair and take shower night before and on the day of the operation without makeup and perfume.
  • Avoid eating anything after midnight the night before facelift (do not even drink water in the morning).
  • Aspirin is prohibited for at least 10 days before surgery and up to three weeks after surgery. These compounds increase the risk of bleeding due to impaired blood coagulation.
  • Be sure to let us know if you smoke or have any other habits.
  • Tell your surgeon if you have a bleeding disease or any acute or chronic illness, even a boil or cold, especially in the week before surgery.

During the Facelift Surgery procedure

The patient can be placed under general anesthesia or deep sleep. Any surgery that is associated with anesthesia has risks, but if the surgeon has more accurate information about the patient’s history and examinations and tests are performed properly and the surgery is performed in equipped clinics and hospitals that are emphasized by the Ministry of Health. Naturally, the risks are minimized. In general, a facelift involves elevating the skin and tightening the underlying tissues and muscles. Fat in the face and neck may be sculpted, removed or redistributed. Facial skin is then covered again over the newly repositioned contours of the face, excess skin is removed, and the wound is stitched or taped closed. The incisions for the procedure depend on the techniques that will be used and the patient’s preferences. Options include:

  • A traditional face-lift incision starts at your temples in the hairline, continues down and around the front of your ears and ends behind your ears in your lower scalp. An incision might be made under your chin to improve the appearance of your neck.
  • A limited incision is a shorter incision that begins in your hairline just above your ear, wraps around the front of your ear, but does not extend all the way into the lower scalp.
  • Neck lift incision starts in front of your earlobe and continues around your ear into your lower scalp. A small incision also is made under your chin.

Post-Operative Care of Facelift Surgery

Postoperative care is very important. Pain, infection and swelling will be minimized if the necessary instructions are followed. Sometimes the side effects after the operation are small so it is not necessary to follow all the tips. Facelift surgery is one of the most powerful methods for facial rejuvenation. No method can remove the chin and sagging neck skin as much as lift surgery.

  1. An adult must accompany the volunteer for at least 24 hours after the operation. Rest after the lift is also essential.
  2. Face and neck dressings will remain for up to 24 hours after surgery.
  3. Up to 2 weeks after the operation, you should rest your head above body level while resting. To do this, use a bed chair or 2 pillows. Try to sleep on your back for 2 weeks after the lift and do not put both sides of your face on the pillow.
  4. It is best to take painkillers before the pain starts. Ibuprofen is generally not recommended for the first three days after treatment because it increases bleeding and bruising.
  5. Minimize daily activities on the day of surgery and a few days after. The volunteer needs 7 to 10 days of rest to resume daily activities.
  6. Apply cold compresses on the treated areas.
  7. Mild bleeding at the wound site is perfectly normal. If cuts on the face bleed, go back to bed, keep your head up, and apply a cold compress with gentle pressure on your face and neck. The main cause of bleeding is bending, lifting objects, coughing, sneezing and other heavy activities.
  8. Avoid taking any medication that raises blood pressure. Remember that even taking 1 aspirin daily is very dangerous and can lead to bleeding. Also, consuming some herbs may increase the amount of bleeding, so they should not be used before or after the lift. Consumption of garlic, ginseng and large amounts of vitamin E can increase bleeding.
  9. Swelling around the eyes, cheeks, face, neck and chest is perfectly normal. This process occurs as a result of the body’s natural response to surgery and complete healing of the affected area. The swelling starts on the day of surgery and peaks 2 to 3 days after surgery, after which it decreases rapidly. Facial skin is stretched due to swelling, which will disappear completely in a few weeks.
  10. Avoid hitting the face, head and neck. Do not hug small babies and sleep alone during the first week after the lift.
  11. Hold the head steady and do not rotate it around, as this will cause the stitches under the skin to rupture.
  12. After general anesthesia, you should drink plenty of fluids. High protein intake and a regular diet are essential for a few days after surgery. Also, drink plenty of water to prevent dehydration. However, adequate water intake prevents nausea and vomiting after the lift operation. Having regular meals will make you feel better and stronger and heal faster.
  13. After the operation, you should wash the wounds 3 times a day with soap and water. This should be done gently and then the desired area should be gently dried. After this, the wounds can be washed with warm water and 50% hydrogen peroxide solution and then some antibiotic ointment can be applied on them. Otherwise, you can use Vaseline because the wounds should not be dry or scaly.
  14. The day after the lift, it is better to take a shower and wash your hair gently with baby shampoo. Of course, when showering and combing your hair, be careful not to damage the stitches. Use a low temperature to dry your hair with a hair dryer as high temperatures can damage wounds. Also, avoid dyeing your hair for up to 6 weeks after the operation. Note, however, that the hair at the site of the wound may be somewhat loose.
  15. The healing process varies from person to person, but it usually takes a year for the wounds to heal.
  16. Starting in the sixth week after surgery, the wound site can be massaged with vitamin E and cocoa butter. The incisions will remain red for several months but will fade over time. Also, after 10 days of surgery, they can be hidden with makeup. Do not expose to direct sunlight for up to 6 months and use sunscreen on the incisions. Such post-lift care is essential and it is best to follow all the tips.
  17. In some cases, the skin changes color after swelling; this is because blood collects under the tissues. This complication usually occurs 2 to 3 days after surgery. Incense can speed up the healing process. Skin discoloration is less common in young volunteers, but the skin of older volunteers, especially the elderly, is more prone to bruising and severe discoloration.

Frequently asked questions

FAQ

Does a facelift require anesthesia?

A Facelift is usually performed under local anesthesia with sedation or under general anesthesia. Your surgeon will be able to advise the best option for you during a consultation.

Who is a good candidate for a facelift?

A Facelift is an option for healthy men and women who are experiencing mid to severe effects of ageing, such as sagging to the cheeks, excess skin on the jowls and deep lines running from the sides of the nose down to the mouth. It should be noted that a facelift is not a solution for the treatment of finer lines and wrinkles, sun damage and hyperpigmentation. Other nonsurgical procedures, such as laser, should be considered either instead or in addition to a Facelift to treat these signs of ageing.

Are the results a facelift permanent?

The results of a Facelift last for approximately ten years. However, having the procedure may help to turn back the visible effects of time but it does not stop the natural ageing process. To ensure that the results look good for as long as possible, you should lead a healthy lifestyle and enjoy a healthy diet, avoid smoking, follow a good skincare routine and avoid excessive exposure to the sun.
