What do the different levels of surgeons mean?

Treatment packages are mainly leveled based on the ranking of surgeons (according to the number of annual surgeries, years of experience, educational and experimental records, the rate of successful surgeries and the percentage of patients’ satisfaction) and the hospital/clinic where the surgery is performed.

As an example, level 1 surgeons are Iranian brand doctors who are famous inside and outside the country and are mainly observed by international patients on social networks. It should be noted that all the doctors and surgeons that cooperate with Nora Health Tour are board certified and work in prominent hospitals that are licensed to accept international patients and comply with the standards of Nora Health Tour medical team.

The most common and most common medical surgery performed on the eye is cataract. Cataract usually occurs in the age range of 65 to 70 years by the aging of the natural lens of the eye and ultimately leading to a gradual clouding of the lens. In individuals with diabetes, cataract is seen five times more frequently. It may can cause blurry vision and increase the glare from lights. Sometimes, people may also have double vision when they look at things or poor night vision and find it harder to drive when it’s dark through the eye with the cataract. Also, when it gets hard to read, work on a computer, and do anything else that calls for clear eyesight.

Causes of a cataract

Uncontrolled diabetesSmokingPast eye surgical history
ObesityCorticosteroid medications due to immune system diseasesHormone therapy
High blood pressureStatin medicines for cholesterol reductionAlcoholism
Ultraviolet radiationHistory of inflammation or injury in eyesFamilial history

Cataracts are diagnosed with an eye examination. In this case, surgery is the only effective treatment for cataract and your doctor may suggest surgery to remove the lens of your eye and replace it with an artificial one which is common and safe procedure, and when it’s done, you’ll be able to see better without needing glasses.

The success rate in cataract surgery is more than 95%. There are a number of different surgical techniques used in cataract surgery In Iran.

Different techniques for cataract surgery

1.   Phacoemulsification:

In this technique, a micro-incision (around 3 millimeters wide) is made at the edge of the cornea. With an ultrasound-based technique called phacoemulsification, the cloudy lens is then extracted through the incision.

2.   Extracapsular surgery:

This technique involves a larger incision (typically 10–12 mm) on the side of the cornea. The cloudy core of the lens is first removed through the opening and the remaining part of the lens is suctioned out. This technique is best for patients who have very hard cataracts or for those unsuited for the phacoemulsification option.

3.   FemtoPhaco:

Also referred to as ‘laser cataract surgery, or ‘laser-assisted cataract surgery’, FemtoPhaco involves a combination of Femto laser treatment and phacoemulsification.

Phacoemulsification (phaco) as a surgical technique, is the most common technique used in Iran and other developed countries. FemtoPhaco also referred to as ‘laser cataract surgery that is performed under intravenous (IV, in a vein) sedation and topical anesthesia (numbing drops inserted into the eye) with femtosecond laser. Femo Phaco Cataract Surgery has a micro incision with no stitch that has fast recovery and you can walk in and walk out on the day of surgery.

Some advantages of laser cataract surgery vs. traditional cataract surgery:

  • Laser incision has been found to seal better than an incision made with a blade, which results in better healing
  • In laser cataract surgery, less energy goes into the phacoemulsification process than with traditional cataract surgery. This reduces the risk of complications, such as damage to the capsule, bleeding, or retinal detachment.3
  • Multifocal lens implants may lessen the dependence on corrective eyeglasses after surgery.

If you have following symptoms you are a good candidate for cataract surgery:

  • Can you see to safely do your job and to drive?
  • Do you have problems reading or watching television?
  • Is it difficult to cook, shop, do yard work, climb stairs or take medications?
  • Do vision problems affect your level of independence?
  • Do bright lights make it more difficult to see?
  • Do you have changes in your color vision or double vision?

Who are NOT good candidate for cataract surgery?

  • There is no special limitation on cataract surgery

Type of lens (IOL: intraocular lens) used to cataracts Surgery

The type of IOL (intraocular lens) implanted used in cataracts surgery in your eye will affect the cost. There are three types of IOL and depends on your ophthalmologist diagnostic and your health situation one of them will be used.

  • Presbyopia-correcting intraocular lenses (P-C IOLs): Corrects nearsightedness or farsightedness
  • Astigmatism-correcting IOLs (A-C IOLs): Reduce or eliminate corneal astigmatism and significantly improve uncorrected distance vision.
  • Multifocal lens implants: Allow you to see at one distance: either far, intermediate, or near.

What to Expect on the Day of Surgery

Cataract surgery itself takes about 20 minutes. But between signing in, getting prepared for your surgery, and postoperative recovery, you can expect to spend between one to three hours. You will go to a pre-operative area where you will have your temperature, pulse, blood pressure, respiratory rate, and oxygen saturation checked. You will have an IV line placed in your arm or hand. Your ophthalmologist and your anesthesiologist might see you before your surgery.

What to expect after cataract surgery

In recovery, you may receive pain medication as necessary. Your nurses will make sure you are comfortable and can walk and eat without difficulty. If you aren’t having any indication of complications (such as pain, bleeding, or swelling), you should be discharged to go home within a few hours after your procedure. Before you are discharged, you will receive instructions about post-surgical eye care, when to make a follow-up appointment, and signs of complications to look out for. .Your vision is expected to improve gradually within a few days after surgery. You may have a cloudy, distorted or blurry vision during the first days following surgery when the eye is healing. It takes a few days for your eye to adjust and get used to the new lens. Normally after a few days, you won’t have much discomfort. Full recovery typically occurs after some eight weeks

Before cataract surgery

You may be instructed not to eat or drink within six to eight hours prior to your cataract surgery, so do not eat breakfast and any food and beverages on the day of surgery. The night before, you should eat light meals and avoid eating milk, eggs, butter, cream and fatty foods.

Your ophthalmologist may also advise you about using medications. Be sure to consult your doctor before taking any medication, including antihypertensive drugs, blood sugar control, blood thinners, etc. Antibiotic eye drops may be prescribed for use one or two days before the surgery. Wash your eyelids and eyelashes with baby shampoo the night before surgery and avoid facial makeup 48 hours before surgery.

Postoperative care after cataract surgery

Your doctor may limit activities, such as bending and lifting, for about a week after your surgery. So it is highly recommended to bring a companion with you for this surgery as it will facilitate your stay and recovery after the surgery. You’ll probably feel a little itching and discomfort for a few days after surgery. You may need to wear an eye patch or protective shield as instructed by your doctor.

  • Do not sleep on your treated eye and ask your ophthalmologist about how long you should sleep in this way during your first postoperative follow-up.
  • 2 days after surgery, you can take a bath, or perform your ablutions.
  • Avoid rubbing and pressing your treated eye, as it may interfere with the normal healing process of the wound and may cause burning and itching of your eye or cause other eye complications.
  • Avoid bending your head for a week.
  • After 6 days, you can prostrate. (*for Muslim patients)
  • Use a protective eye shield at least for a week at night while sleeping.
  • Ask your ophthalmologist when you are allowed to wear eye makeup, drive and swim.
  • Avoid lifting objects more than 10 kg in the first few weeks after surgery.
  • If your ophthalmologist has prescribed a protective eye shield or eye wear for you, be sure to wear it to protect your eye against possible eye injuries.
  • There is no need to change your diet after surgery. Of course, it is best to use soft meals in the first hours after surgery. Also avoid constipation by eating the right amount of Carrots and other red, yellow, orange, and dark-green leafy vegetables, fruits and water. Avoid sugar and keep sweet foods to a minimum. Sugar prevents white blood cells from being their strongest.
  • Take your eye drops and medications according to your ophthalmologist’s direction to reduce inflammation, prevent infection, and control eye pressure.

Finally your ophthalmologist will clarify the healing process in order you to have no worries at all.
